Secondary Application Instructions (Registration for new students only)

Application Instructions for students entering Grades 7-12 during the 2012-2013 school year:  Fill out the Application and Laptop Use forms completely.  Type or clearly print in black or blue ink.

•  Student Enrollment Form submitted online
• Laptop Use Form
• Copy of Student’s Birth Certificate
• Proof of Internet Access – Provide a copy of your high-speed Internet bill.
• Unofficial Transcript – A copy may be obtained from the student’s current school.  Do not disenroll your child from their current school.
• Copy of First or Second Quarter Report Card – A copy may be obtained from the student’s current school.
• Copy of the Student’s most recent Standardized Test Scores – A copy may be obtained from the student’s current school.  For example, Hawaii State Assessment (HSA), Stanford Achievement, Iowa Test of Basic Skills (ITBS), California Achievement.
